Brings in: ACP SDK 1.1 + dev tools (agentclientprotocol#257), boolean Fast-mode config options (agentclientprotocol#255), message IDs on text session chunks (agentclientprotocol#256), goal changes as session metadata (agentclientprotocol#263), completed image-generation items (agentclientprotocol#262), codex 0.142.5 (agentclientprotocol#261), vscode-jsonrpc v9 (agentclientprotocol#258). Fork patches preserved (outputSchema turn-level forwarding, base/developerInstructions session overrides) — targeted suites pass. Version -> 1.4.0.
